Alignment Health Expands Medicare Advantage to San Antonio for 2027
Alignment Health has announced an expansion of its Medicare Advantage coverage into the San Antonio region, specifically targeting Bexar and Comal counties beginning January 1, 2027. This expansion introduces new healthcare options and coordinated care models for more than 392,000 Medicare beneficiaries residing across these two counties.
San Antonio marks Alignment’s second market in Texas, following its established footprint in El Paso. With this new addition, the company’s total Texas service area now spans four counties and serves nearly 550,000 Medicare beneficiaries.
2027 Texas Plan Portfolio and Local Provider Networks
For the 2027 plan year, Alignment Health offers a comprehensive portfolio of 10 Medicare Advantage plans across the San Antonio and El Paso regions, supported by a network of over 2,000 physicians and specialists.
Members in the newly added Bexar and Comal counties will gain in-network access to several prominent local health systems and medical groups, including:
- Baptist Health System
- Genesis Medical Group
- Hillside Primary Care
- Gonzaba Medical Group (Bexar County members)
- Harlandale Clinic (Bexar County members)
Meanwhile, members in the El Paso and Hudspeth counties will continue to access local providers such as The Hospitals of Providence, Providence Medical Partners, HCA Healthcare, and community practices like Centro San Vicente.
Quality Ratings and Specialized Plan Options
Alignment’s Texas HMO contract earned a 4.5 out of 5-star rating from the Centers for Medicare & Medicaid Services (CMS) for 2027. The annual CMS rating system evaluates health plans on key performance measures, including member experience, customer service, and care coordination.
The 2027 portfolio introduces specific plan choices tailored to different healthcare and financial requirements:
- Alignment Health ONE 011 / ÚNICO 011 (HMO-POS): Designed to provide comprehensive coverage in Bexar and Comal counties.
- Alignment Health Smart 012 (HMO-POS): Offers members a monthly rebate toward their Medicare Part B premium.
- Specialized Plans (C-SNP / D-SNP): Tailored options for qualifying individuals living with specific chronic conditions—such as diabetes or certain heart conditions—or those eligible for both Medicare and Medicaid.
- Alignment Health ONE + Walgreens 001 (HMO-POS): Continues in El Paso and Hudspeth counties with exclusive over-the-counter access through Walgreens.
Member Support and Annual Enrollment Period
All members have access to a 24/7 concierge care team designed to help navigate benefits, locate in-network physicians, and coordinate medical care. Depending on the chosen plan, additional benefits may include prescription drug copays, over-the-counter and grocery allowances, fitness programs, and in-home support.
